Safe, compassionate care for frail older people using an integrated care pathway

This practical guide for commissioners summarises evidence of the effects of an integrated pathway of care for older people and suggests it can be commissioned effectively using levers and incentives across providers. If frail older people are supported to live independently and educated to manage their long-term conditions effectively, they are less likely to reach crisis, require urgent care and experience harm. A task and finish group, part of the Commissioning Development work stream, a work stream of the Community Nursing Strategy Programme is working to support implementation across the system.
